DESCRIPTION OF OCCURRENCE:
On 12/22/14 male patient (A), in his 50s, pushed male patient (B)causing him to hit his head. Patient (B) was in his 60s.
The facility conducted an internal investigation. The facility notified the police department. Patient (A) was talking to another patient. Patient (B) made a comment to patient (A). Patient (A) pushed patient (B) causing patient (B) to fall to the floor and hit his head on the door frame. Patient (A) was placed in a manual hold and escorted to locked door seclusion. Patient (B) was assessed and transferred to the hospital for further evaluation. A CT scan indicated there were no intercranial abnormalities. Both patients had a mental health diagnoses and a history of behavior problems which had been care planned. Patient (A) was placed on assault precautions and patient (B) was moved to another wing on the unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an internal investigation, separating the patients, assessing patient (B), transferring patient (B) to the hospital for further evaluation, placing patient (A) on assault precautions, and moving patient (B) to another wing on the unit.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
