DESCRIPTION OF OCCURRENCE:
On 12/10/14 male patient (A) was struck by male patient (B). Both patients were in their 20s.
The facility conducted an internal investigation. The facility notified the police, and appropriate administrative personnel. Patient (B) had a history of behavior problems and a treatment plan was in place. The patients were lined up to return from the gym. Patient (A) stated that he told patient (B) that he had cut in line. Patient (B) hit him several times with a closed fist. Staff placed patient (B) into a protective restraint and escorted him back to the unit. He was then placed in seclusion. Patient (A) was assessed and noted to be bleeding from his nose, and he had a scratch on his cheek. Pressure was applied to stop the bleeding. The scratch was cleaned and a bandage applied. Patient (B) admitted to hitting patient (A) in an attempt to be returned to jail. The allegation was substantiated as the incident was witnessed by staff. Patient (B) was placed on assault II precautions.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, placing patient (B) in seclusion, assessing and treating patient (A), and placing patient (B) on assault II precautions.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
