DESCRIPTION OF OCCURRENCE:
On 11/18/14 male patient (A), in his 30s, struck male patient (B), who was also in his 30s.
The facility conducted an internal investigation. The facility notified the police department. The two patients began arguing back and forth. Patient (A) hit patient (B) in the face twice before staff intervened and escorted patient (A) to the seclusion room. Patient (B) was escorted to the treatment room. He was assessed and noted to have a laceration above his right eye. Patient (B) was transferred to the hospital for further evaluation and treatment. The laceration was cleaned and sutured. Both patients had mental health diagnoses and a history of behavior problems which had been care planned. Patient (A) was placed on assault ll precautions.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, separating the patients, placing patient (A) in locked door seclusion, assessing and treating patient (B), transferring patient (B) to the hospital for further evaluation and treatment, and placing patient (A) on assault ll precautions.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
