DESCRIPTION OF OCCURRENCE:
On 11/14/14 male patient (A), in his 20s, struck male patient (B), who was in his 30s.
The facility conducted an internal investigation. The facility notified the police department. Patient (B) was socializing with his peers. Patient (A) began swearing and calling patient (B) names. Patient (A) lunged at patient (B) and hit him in the face. Staff intervened and pulled patient (A) off. Patient (A) was placed in locked door seclusion. Both patients had mental health diagnoses and a history of behavior problems which had been care planned. Patient (B) was assessed and noted to be bleeding from his lower lip and had a small scratch on the side of his nose. The injuries were cleaned and treated. He denied being fearful of patient (A). Patient (A) was placed on assault ll precautions and moved to another wing away from patient (B).
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an internal investigation, separating the patients, assessing and treating patient (B), placing patient (A) in locked door seclusion, placing patient (A) on assault ll precautions, and moving patient (A) to another wing.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
