DESCRIPTION OF OCCURRENCE:
On 11/13/14 male patient (A), in his 20s, was involved in a physical altercation with male patient (B). Patient (B) was in his 40s.
The facility conducted an internal investigation. The facility notified the police department. The two patients were sitting together in the day hall. Patient (A) suddenly stood up and started yelling at patient (B). Patient (B) stood up and they began hitting each other. Patient (A) was placed in a restraint hold and escorted to his room. Patient (B) was taken to the treatment room. He was assessed and noted to have a bloody nose, a small laceration on his lower lip and swelling of the right eye. Pressure was applied to stop the bleeding from the his nose. Vital signs and a neurological assessment were obtained. The cut on his lip was cleaned and an ice pack was applied to the right eye area.
X-rays were negative for fracture. Both patients had mental health diagnoses and a history of behavior problems. The two patients were placed on assault ll precautions and peer restrictions from each other. The patients reside on separate wings of the unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an internal investigation, separating the patients, assessing and treating patient (B), placing patient (A) in a restraint hold, obtaining an x-ray on patient (B), and placing both patients on assault ll precautions and peer restriction.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
