DESCRIPTION OF OCCURRENCE:
On 11/12/14 male patient (A), in his 20s, and male patient (B), in his teens, were involved in a physical altercation.
The facility conducted an internal investigation. The facility notified the police department. Staff observed the two patients hitting each other with closed fists on the face and chest. Staff intervened and separated the patients. Patient (B) was escorted to the seclusion room and assessed. He had a laceration on his lower lip, lacerations on the knuckles of both hands and slight swelling on his right cheek. The laceations were cleaned and treated and vital signs assessed. Patient (A) did not sustain any visible injury. Both residents had mental health diagnoses and a history of behavior problems which had been care planned. Patient (A) and patient (B) were placed on assault ll precautions. They reside on separate wings of the unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, separating the patients, assessing and treating patient (B), and placing the patients on assault ll precautions.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
