DESCRIPTION OF OCCURRENCE:
On 11/11/14 male patient (A), in his 60s, was involved in a physical altercation with male patient (B), also in his 60s.
The facility conducted an internal investigation. The facility notified the police department. Staff heard yelling in the hallway. They observed the two patients sitting close to each other with their legs intertwined. Patient (B) was noted to have scratches above his right eyebrow and the tip of his nose. The injuries were cleaned and treated. His glasses were also broken. Patient (B) stated he hit patient (A) because patient (A) shoved him. Patient (A) did not have any visible injuries. The patients were separated. Both patients had mental health diagnoses and a history of behavior problems. The patients were placed on assault ll precautions and counseled to stay away from each other and they both agreed.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, separating the patients, interviewing patients and staff, assessing and treating patient (B), placing both patients on assault ll precautions, and counseling both patients.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
