DESCRIPTION OF OCCURRENCE:
On 11/02/14 female patient (A), in her 20s, and female patient (B), also in her 20s, were involved in a physical altercation.
The facility conducted an internal investigation. The facility notified the physician and the police department. Patient (B) was approaching the medication window. Patient (A) walked up to patient (B) and grabbed her hair. The two patients began punching each other. Staff intervened and separated them. Both patients had mental health diagnoses and a history of behavior problems which had been care planned. The patients were assessed. Patient (A) had several scratches on her neck and complained of a bump on the right side of her head. Vital signs were within normal limits. Patient (B) did not have any visible injury and refused to participate in an interview. Staff escorted patient (A) to the seclusion room for safety. She was placed on assault ll precautions.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an internal investigation, separating the patients, assessing and treating patient (A), interviewing patient and staff, placing patient (A) in seclusion with assault ll precautions, and updating care plans.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
