DESCRIPTION OF OCCURRENCE:
On 10/30/14 male patient (A), in his 20s, struck male patient (B), who was in his 30s.
The facility conducted an internal investigation. The facility notified the police department. A staff member witnessed patient (A) punch patient (B) in the face while in the fenced yard area. Staff intervened and separated the patients. Patient (A) walked to the seclusion room and was placed in locked door seclusion. Both residents had mental health diagnoses and a history of behavior problems which had been care planned. Patient (B) was assessed and noted to have a small laceration and swelling to his lower lip. The injury was cleaned and ice applied. He denied being fearful of patient (A). The patients stayed in separate wings of the unit. Patient (A) was placed on assault ll precautions.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an internal investigation, separating the patients, placing patient (A) in locked room seclusion, assessing and treating patient (B), interviewing patients and staff, and placing patient (A) on assault ll precautions.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
