DESCRIPTION OF OCCURRENCE:
On 10/27/14 male patient (A), in his teens, pushed, kicked and punched male patient (B) also in his teens.
The facility conducted an internal investigation. The facility notified police. As patient (A) walked past patient (B) in the day hall, he kicked and pushed patient (B) into the corner and then began to punch him several times in the face and head. Staff separated them. Patient (A) was walked to the locked intensive observation area on the unit. Patient (B) was assessed and had a nose bleed that stopped when pressure was applied. He also complained of pain to his head that he rated 10 out 10. A ½ inch bump was noted on the occipital area of his head. He was alert and oriented and neurological checks and vital signs were within normal limits. An ice pack was applied to his head and Tylenol was administered for pain. Earlier in the day, patient (B) had been making derogatory remarks toward patient (A). The patients were both placed on assault II precautions. They were placed in programming opposite of each other and were being housed on separate floors of the unit.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, assessing and treating patient (B), placing both patients on assault precautions, keeping them apart in programming and placing them on separate floors of the unit.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
