DESCRIPTION OF OCCURRENCE:
On 10/27/14 male patient (A), in his 20s, physically assaulted male patient (B), who was also in his 20s.

FACILITY ACTION:
The facility conducted an internal investigation. The facility notified the physician and the police department. Patient (A) had been agitated and refused to take his medications. He walked down the hall to his room and stood outside the door. Staff offered him several options including a voluntary time out in his room, but he refused. Suddenly, he entered patient (B's) room and started attacking him. Staff responded and separated them. Patient (A) was placed in seclusion. Patient (B) was assessed and noted to have a small bruise on his forehead. Both residents had mental health diagnoses and a history of behavior problems which had been care planned. Patient (A) was placed on assault ll precautions and was on a separate wing away from patient (B).

DEPARTMENT FINDINGS:
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an internal investigation, separating the patients, placing patient (A) in seclusion, assessing patient (B), and placing patient (A) on assault ll precautions away from the other patient.

No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
