DESCRIPTION OF OCCURRENCE:
On 10/21/14 male patient (A), in his 30s, struck male patient (B) who was also in his 30s.
The facility conducted an internal investigation. The facility notified the physician and the police department. Patient (A) exited the shower area and approached patient (B) from behind and hit him on the right side of his head. Patient (B) was assessed and noted to have a laceration to his right ear. Staff contained patient (A) and placed him in seclusion and restraint. Both patients had been diagnosed with mental health issues and had a history of behavior problems which had been care planned. Patient (B) was treated and transferred to the hospital for further evaluation and treatment. He indicated he wanted to press charges against patient (A). Patient (A) was placed on assault ll precautions and emergency medications were initiated.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an internal investigation, assessing and treating patient (B), placing patient (A) in seclusion and restraint, transferring patient (B) to the hospital for further evaluation and treatment, placing patient (A) on assault ll precautions, and administering emergency medications to patient (A).
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
