DESCRIPTION OF OCCURRENCE:
On 10/15/14 male patient (A), in his 20s, hit male patient (B) in the eye. Patient (B) was in his 40s,
The facility conducted an internal investigation. The facility notified police and the physician. The patients were in the day hall. Patient (A) hit patient (B) in the eye. Staff separated them and police escorted patient (A) to his room. Shortly after that, patient (B) attempted to run into the wing where patient (A) was in an attempt to assault him. Staff intervened before he could make contact and walked patient (B) to the day hall. The physician examined patient (B). His right eye had a laceration above the eyebrow. The area was cleaned and a Band-Aid was applied. Both patients were placed on assault II precautions and a peer restriction. Patient (A) was moved to a different wing of the unit to separate the 2 patients. Patient (B) was making threats toward staff and peers after the assault. He had decompensated, was expressing paranoid ideations and was started on emergency medications.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, assessing and treating patient (B), moving patient (A) to another unit and treating patient (B) with emergency medications.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
