DESCRIPTION OF OCCURRENCE:
On 10/12/14 patient (A) grabbed patient (B's) hair and began hitting patient (B) in the face and head. Both patients were in their teens.
The facility conducted an internal investigation. The facility notified police and the families/guardians. The patients were in the dining room. Patient (A) came up behind patient (B), grabbed her hair and began hitting her. Staff separated them. Patient (A) was escorted to the Intensive Evaluation Treatment area for time out. Patient (B) was assessed and reported pain to her face and head. She had a cut under her lip and a cut above her right eye. There was swelling to her lip and left cheek. She was transferred to the hospital and cleared medically. She was admitted to that hospital's adolescent psychiatric unit at her family's request. Patient A was placed on assault II precautions with staff monitoring.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, separating the patients, assessing patient (B) and transferring her to the hospital, and placing patient (B) on assault precautions.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
