DESCRIPTION OF OCCURRENCE:
On 10/12/14 male patient (A), in his 30s, and male patient (B), also in his 30s, were involved in a physical altercation.
The facility conducted an internal investigation. The facility notified the physician and the police department. Patient (B) walked over to patient (A) on hit him on the head. The two patients began to fight and staff attempted to separate them. Patient (A) was able to bite patient (B) on his right forearm. Staff contained patient (A) and he was escorted to his room for a time out. Both patients had mental health diagnoses and a history of behavior problems which had been care planned. The patients were assessed and patient (B) was noted to have a superficial bite mark on his right upper forearm which was treated. Patient (A) was placed on assault ll precautions. Staff will monitor both residents closely .
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an internal investigation, separating the patients, assessing and treating patient (B), interviewing patients and staff, placing patient (A) on assault ll precautions, and monitoring the patients closely.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
