DESCRIPTION OF OCCURRENCE:
On 09/17/14 male patient (A), in his 30s, and male patient (B), in his teens, were involved in a physical altercation.
The facility conducted an internal investigation. The facility notified the physician and the police department. Staff observed patient (A) entering patient (B's) room and then heard a loud noise. Staff entered the room and observed the two patients fighting. Patient (B) was removed from the room. Patient (A) was moved to another room in a separate wing away from patient (B). Both patients had a history of behavior problems which had been care planned. The patients were assessed and patient (B) had a laceration on the inside of his upper lip. He was treated with pain medication. Patient (A) had an abrasion under his left eye with swelling to his left eye and upper lip. He refused any treatment. Patient (A) was placed on assault II precautions.
The Department reviewed the facility's report and supplemental documentation and found that the facility acted appropriately by reporting the occurrence, notifying the appropriate persons and agencies, conducting an investigation, assessing and treating the patients, interviewing patients and staff, moving patient (A) to another room, and placing patient (A) on assault II precautions.
No deficient practice was cited. The Department will review this occurrence prior to any survey or upon receipt of any complaint that may be filed against this facility.
